Photo Release Form

I hereby grant permission for First Commonwealth Bank and its representatives to take and use photographs, videos, or other media of me or my likeness for the purposes of marketing, advertising, promotion, or other related business activities.  If I am completing this form on behalf of a minor child, references to “me” or “my” refer to the child identified below.

By typing my full name below and clicking the "Submit" button, I consent to the following:

1.    Authorization: I authorize First Commonwealth Bank to use my photograph, image, video, or likeness in any form and for any lawful purpose, including but not limited to promotional materials, website content, social media platforms, and advertisements.

2.    Waiver of Approval Rights: I understand that I will not have any right to inspect or approve the final versions of the photographs or videos, or the uses to which they may be applied.

3.    Compensation: I acknowledge that I will not receive any compensation, payment, or royalty for the use of my image or likeness.

4.    Duration of Use: I understand that my image or likeness may be used for an indefinite period of time, and that the permission I am granting is perpetual.

I hereby acknowledge that I have read and fully understand the terms of this photo release form and that I am voluntarily consenting to the use of my image or likeness as described above.  If I am completing this form on behalf of a minor child, I confirm that I am the parent or legal guardian with authority to complete this form on the child’s behalf.
